England Hockey Participation Agreement

With return to hockey now in step 4 there is a requirement to sign the Participation Agreement with England Hockey.

Senior Player - Complete for yourself
Juniors - Players under 18 to be completed by parent/guardian

Players who have not signed up will not be allowed to play.
